IT Refinement

Work is currently underway on scoping up closer integration of CCM IT tools within MedTech. This project will allow for CCM workflow to be more tightly integrated into MedTech, allowing seamless integration with electronic clinical decision support. The architecture of this work will be supported only via the Health Network.

Pricing model implementation

After consultation with our PHO’s, the CCM pricing model has been redefined to allow for business simplification at both the PHO and GP practice end. There have been some adjustment to patient services, including the introduction of CVD Get Checked Alignment for patient who have exited 12 months on the CCM CVD programme. Implementation will not only be ensuring the IT can accommodate some of the changes, that CMDHB reporting will allow us to maintain a fee-for-service or a capitation option for PHOs.
